Epic Staffing Group (“Epic,” “we,” “us,” or “our”) collects personal information from you for business purposes, including evaluating your qualifications, responding to your inquiries, providing staffing services, and supporting our operations in the United States and the Philippines. This Notice at Collection explains the categories of personal information we collect, the purposes for which we use it, whether we sell or share it, and how long we retain it.
This Notice applies at the point where you interact with any Epic form, application, website, mobile app, job board, credentialing portal, or data intake system.

Depending on your interaction with Epic (candidate, employee, clinician, referral, website visitor, client), we may collect:
A. Personal Information
- Contact details (name, email, phone, address)
- Account information and login credentials (if applicable)
- Employment history, resume, job qualifications
- Education and certifications
- Professional license numbers and expiration dates
- Work eligibility documentation
- Background check information
- Communication records
B. Sensitive Personal Information (SPI)
We may collect the following SPI when necessary for staffing, placement, onboarding, credentialing, payroll, or compliance:
- Social Security Number
- Driver’s license, passport, or government ID
- Financial account or payroll information
- Health and vaccination information
- Demographic information you voluntarily provide
- Biometric identifiers (if required by background vendors)
C. Automatically Collected Information
- IP address and device identifiers
- Browser and device information
- Cookie and tracking data
- Usage metrics from our website or job search app
2. Purposes for Collection and Use
We use personal information to:
- Evaluate candidates for job opportunities
- Conduct credentialing, verification, and compliance checks
- Manage the employment or engagement relationship
- Process payroll, benefits, timekeeping, and assignments
- Communicate with you regarding opportunities or services
- Operate, secure, and improve our websites, systems, and apps
- Comply with legal, contractual, and regulatory requirements

Sensitive Personal Information is used only for permitted business purposes and not for inferring characteristics about you.
3. Selling or Sharing Personal Information
Epic does not sell personal information.
Epic may “share” personal information for cross-context behavioral advertising to the extent tracking technologies (e.g., analytics, cookies, pixels) are used.

5. Retention of Personal Information
We retain personal information only as long as necessary for the purposes described above, or as required by law.
General guidelines:
- Candidate records: 4 years from date of decision
- Employee records: 7 years from termination of employment, or longer where required to comply with wage/hour, discrimination, contract, or benefit-related legal requirements.
- Credentialing documentation: 7 years from termination of employment, or longer where required to comply with wage/hour, discrimination, contract, or benefit-related legal requirements.
- Payroll/tax information: 6 years, subject to a longer retention period as necessary to support the calculation of a retirement or pension benefit as discussed below
- Website and app analytics: 26 months
